Eye Exams: Steve Sisler, DVM, DACVO, from Eye Care For Animals, Wheeling, IL. Dogs must be over 7 weeks of age.  CERF forms available.  Please bring AKC registration & permanent ID information.  Cost $28 pre-registration*, $30 on site.

 

Optigen (CSNB): 20/20 clinic with satellite. Reduced fee - 20% off regular price - see Optigen website http://www.optigen.com  Pre-registration and payment directly to Optigen is absolutely required.  Test request forms should be completed online using code NSDTR92206  for an additional 5% discount.  Forms may also be completed on site.  Blood samples will be drawn at the clinic for a $5 fee.  You may also register on line at Optigen and have your local vet draw and ship the blood. Samples must be received at Optigen the week of September 25th.

 

Microchipping: Home AgainŽ microchip provides permanent identification. Insertion of the chips will be available at a cost of $25 each. Registration forms will be provided and require a one-time additional registration fee to Home AgainŽ to be submitted by the owner.
DNA Collections:  

 

AKC DNA profiles:

Cheek swabs and registrations forms will be available on site at a discounted price of $30. The BMT Medical Committee will assist with your swabbing at no charge.

 

CHIC DNA Repository:

Your dog must be pre-registered with CHIC at www.caninehealthinfo.org/dnabank/html. Bring the sample kit that you will receive from CHIC and have your dog's blood drawn for $5. WE will help you with your cheek swabs also. There is an additional fee to CHIC to submit the samples.
